8/24/26 - The District is inviting the submission of Request for Proposals for snow plowing and/or removal at the Sandy Creek Central School District for the 2026-2029 Winter Seasons. Proposals will be accepted until noon on the 17th day of September, 2026 at the District Clerk's Office. Anyone with questions, or to view the area can contact Andy Ridgeway at the District Office at 315-387-3445 ext. 1510.

Welcome to the Sandy Creek School Based Health Center

School-Based Health Center Services – Sandy Creek Central School District
The School-Based Health Center (SBHC) provides prompt, convenient, and high-quality healthcare to enrolled students in the Sandy Creek Central School District. Operated by ConnextCare and supported by state funding, these centers are an extension of our commitment to keeping students healthy and ready to learn.
There are no out-of-pocket costs for services provided at the SBHC. We bill most insurances and accept what we receive as full payment. However, you may be financially responsible for certain services that fall outside of the SBHC, such as:
- Prescriptions
- Laboratory tests
- Referral appointments

If your child does not currently have health insurance, ConnextCare’s Outreach Coordinators can assist you in exploring coverage options.

Hours of Operation
The SBHC is open during regular school hours and follows the school calendar. When the center is closed, patients are encouraged to call the Pulaski Health Center at 315-298-6564 for medical assistance.

- Diagnosis and Treatment for Acute Illness and Minor Injuries
- Immunizations
- Allergy Injections
- Comprehensive Physicals - Well Child Checks, Sports Physicals, Working Paper Physicals

Dental Services (with separate dental enrollment)
- Dental Cleanings
- Fluoride Treatments
- Sealants
- Dental X-rays
- Oral Health Education

Enrollment
To enroll your child, simply complete an enrollment form for each student. Forms are available:
- At the SBHC
- In the school's main office
- Online at www.connextcare.org

Enrolled students are typically seen the same day an appointment is made.
