School Based Health Center

Information
The School Based Health Center has been serving the health care needs of enrolled students in the district since 1996. The center is a state funded health care facility, and is an extension of ConnextCare. Care is provided to enrolled students with either insurance billing, or with no fee should the pupil not be covered by an insurance plan.

The center's hours of operation follow the school schedule, as well as being open for business on a reduced schedule in the summer months. When the center is closed, patients are encouraged to call and use the ConnextCare Pulaski location at 298-6564 for their health care needs.

Among the many services available from the School Based Health Center, with Parent/Guardian permission are:

Diagnosis and treatment for illness
• Immunizations
• Allergy shots
• Comprehensive Physicals
• Referrals to counseling
While services at SBHC are at no cost to you, there are some costs that you would be responsible for should any of the below services be performed following your permission:

• Laboratory Services
• X-ray scheduling
• Prescriptions-Called to patient chosen pharmacy
• Specialty Appointments
Enrolled students are usually seen the same day as an appointment is made. Please call the School Based Health Center at 387-3620 to enroll, make an appointment, or with any questions. Your concerns will be addressed promptly. We look forward to hearing from you!
